0165(2)(c) Cords were observed hanging from the TV and running along the floor line accessible to children in the licensed Livingroom space. 0165(3)(g) Fence in outdoor space surrounding children's play area was observed to have flaking paint on upper railing. Also, it was observed that several toys were broken, craked or in poor condition, including: A Racetrack, a green tote, and 2 toy cars with cracks. 0165(4)(c) Fan light fixture in licensed Infant room was observed to be missing shatter resistent bulbs. 0165(4)(e) Water temperature observed during monitor was 122.3. 0165(5)(b) Electrical outlet in the licensed bathroom near the sink was observed to be missing being GFCI type.
